What is instrumental esophageal injury?

After esophagoscopy, the patient suffered from swallowing pain, neck tenderness and subcutaneous emphysema. After dilation, there are signs of infection such as retrosternal or back pain or elevated body temperature. In severe cases, cyanosis, shock and death may occur.

X-ray examination showed that there was emphysema between the fascia on the lateral film of the neck. The retropharyngeal space widened and normal cervical curvature disappeared. Chest X-ray film of thoracic esophageal rupture showed widened mediastinal shadow, emphysema and pleural effusion on the affected side.

X-ray or CT examination of neck or chest can be used. X-ray contrast esophagography is helpful in the diagnosis of most cases, but negative contrast cannot completely exclude perforation. Acute abdominal pain due to other causes, such as intramural aneurysm, myocardial infarction or pulmonary embolism, should be differentiated.
