summary

When I was about to go to work this morning, a family member of my patient was waiting at the door of the office. She said that she wanted to ask me some questions, because the patient was about to have an operation, so she was worried. Let's talk about how to operate on esophageal cancer.

How does esophagus cancer operate

First, esophagectomy is a surgical method for the treatment of esophageal cancer, which has strict selection according to the trial situation. Esophagectomy is suitable for the following cases: early esophageal or cardiac cancer, and part of the third stage lower esophageal cancer. The lesion length is less than 5cm, the general condition is good, there is no distant metastasis, and there is no serious damage to heart, lung, liver, kidney function or other surgical contraindications.

The second is esophagogastrostomy through the left thoracic aorta arch. Choose the body position, make the incision, choose the right lateral decubitus position and the left thoracic posterolateral incision. Remove the sixth rib, cut off the posterior segment of the fifth rib at the same time, and enter the chest through the rib bed. The size, activity, relationship with surrounding organs and local lymph node metastasis of the tumor were carefully explored.

Third, if the tumor has invaded the hilar organ or aorta, or has extensive lymph node metastasis, it is not suitable for resection. If the tumor has a certain degree of activity, without the above signs, then cut the mediastinal pleura longitudinally, separate the lower pulmonary ligament, reach into the mediastinal incision and try to pick it up; if the mass moves with the finger, and can slide between the hilar, aorta and spine, it means that the tumor has not invaded, it can be removed.

matters needing attention

In general, if the physical condition of patients with esophageal cancer is still allowed, surgery is still adopted, because only surgery can completely remove the focus, but some patients are not able to accept surgery, so radiotherapy and chemotherapy can be considered at this time.
