The granulation tissue was surrounded by fibrous capsule and proliferated by cord like or reticular epithelium.

Periapical cyst: usually without conscious symptoms, the cyst enlargement makes the jaw wall thin and bulge, palpation has ping-pong like feeling. X-ray showed that there was a well-defined bone sparse area around the apex, and there were obvious white lines around it. The rupture and infection of cyst can form sinus.

Periapical abscess or chronic alveolar abscess: chronic suppurative inflammation confined to periapical area. There were mainly polymorphonuclear leukocytes and monocytes in the pus, and there were dense lymphocytes and plasma cells around them. Periapical abscess can pass through alveolar bone and mucosa to form gingival sinus, or through skin to form skin sinus. Generally, there were no conscious symptoms, slight pain and a history of repeated swelling during percussion.
