Can membranous glomerulonephritis be cured?
Under light microscope, mesangial cells and matrix proliferated. The mesangial area was widened with neutrophil infiltration. The glomeruli were enlarged and the cells increased. The glomeruli were lobulated due to the proliferation of mesangial cells and the increase of mesangial matrix. The glomerular basement membrane was obviously thickened, and the processes of mesangial cells inserted into the adjacent capillary loops and formed mesangial matrix, resulting in the separation of basement membrane. PAS staining was double track. Some glomeruli may have crescent formation. In the late stage, mesangial and glomerular fibrosis, sclerosis, the formation of the whole glomerulus without structure of the glass like mass. The corresponding renal tubular atrophy, interstitial fibrous tissue hyperplasia, there are many lymphocytes and monocytes infiltration.
Electron microscope and immunofluorescence: according to its ultrastructure, it can be divided into type I and type II. Type I is more common, mainly in the subendothelial dense deposits, mesangial area and sometimes subcutaneous deposits. Immunofluorescence showed the deposition of C3 particles. Type II: irregular banded deposits with high electron density appeared in the dense layer of glomerular basement membrane.
Clinicopathological connection: early symptoms are not obvious, clinical symptoms are different, often proteinuria, hematuria. About half of the patients show nephrotic syndrome, and often have hypertension and renal insufficiency. In the late stage, the whole glomerulosclerosis has a long course of disease. The effect of hormone therapy is not obvious, and it is easy to turn into renal failure. Some patients have lower serum complement, so it is also called hypocompleminemic glomerulonephritis.
