summary

Cerebral micro infarction is another pathological manifestation of cerebral small vessel disease, which brings great harm to patients. It is a cerebral infarction focus that can not be seen only by histopathology with naked eye. It is very common in autopsy of brain, especially in patients with dementia or other manifestations of ischemic cerebrovascular disease. Now let's talk about how to check lacunar cerebral infarction?.

How does lacunar sex cerebral infarction check?

First: neuropathological examination: Although the spatial and temporal resolution of neuropathological examination is very high, it can find different stages of cerebral micro infarction (as shown in Figure 1), but the detection method is invasive, and the brain tissue coverage is very low. Fig. 1 neuropathological examination: hematoxylin eosin stained sections of the infarcted brain. A shows the acute MI in the middle frontal gyrus (indicated by the arrow) and the red hypoxic neurons (indicated by the arrow in the small picture); B shows a chronic slit like microinfarct in the gyrus fold of the inferior parietal lobule cortex (arrow); C shows chronic MI with cavity formation in the inferior parietal lobular cortex (arrow); D shows a chronic MI in the inferior parietal lobular cortex with hemosiderin deposition (arrow in the small figure).

Second: diffusion weighted MRI diffusion weighted MRI imaging (DWI) can be highly sensitive to new cerebral micro infarction, can find the diameter of 1-2 mm of cerebral infarction, but more than 2 weeks of infarction exist DWI signal reduction, so the time resolution is low. The outcome of lesions with high signal intensity on DWI at 3 months is uncertain. Previous data suggest that these lesions may be cavitary, or T2 high signal intensity without cavitary, or imaging is not obvious.

Third: high resolution MRI high resolution MRI makes a big step forward in the study of cerebral micro infarction, because it can clarify the etiology and outcome of these small ischemic lesions. Previous radiology histopathology association studies have found that high field intensity 7T MRI can detect 1-2 mm cerebral micro infarct lesions, and is not limited to acute or subacute phase.

Some studies also suggest that cerebral micro infarction may affect other vascular damage, markers of nervous system degeneration and motor function damage in varying degrees by destroying brain network circuits.
