What is abdominal compartment syndrome?
summary
Ventricular septal syndrome (VSS) refers to the performance of tissue function and circulation disorder caused by elevated pressure in a limited space. Ventricular septal syndrome often occurs in the compartment of limb fascia, which is called osteofascial compartment syndrome. If it occurs in the eyeball, it is called glaucoma. If it occurs in the cranial cavity, it is called intracranial hypertension syndrome. Let me share my experience with you.
What is abdominal compartment syndrome?
1. Abdominal distension and abdominal wall tension are the most direct manifestations of abdominal hypertension caused by increased intra-abdominal volume. Open decompression showed that the bowel was highly edematous, gushing out of the incision, and the bowel could not be returned after operation. The increase of peak inspiratory pressure > 8.34kpa (85cmh2o) was the result of diaphragmatic elevation, increased thoracic pressure and decreased lung compliance.
Oliguria is caused by insufficiency of renal blood perfusion and increase of aldosterone and ADH. For fluid resuscitation, dopamine and furosemide did not increase urine volume. Refractory hypoxemia and hypercapnia can * t provide sufficient alveolar ventilation because of mechanical ventilation, and decrease arterial oxygen partial pressure and CO2 retention.
So far, there is no comparative study of temporary "abdominal closure" technique, so clinical reports are based on personal experience. Although it has been reported in many literatures that continuous suture of skin with 20-30 cloth towel forceps or single strand nylon thread after enlarging median incision without fascia suture can prevent abdominal hypertension, in a few cases, if the decompression is not enough, the intra-abdominal pressure can still reach 6.66kpa (50mmhg) or higher.
matters needing attention
Close observation of abdominal signs and systemic changes is the key to discover abdominal compartment syndrome. If abdominal distension and abdominal wall tension occur, and organ dysfunction occurs, timely open decompression can also reduce the mortality of abdominal compartment syndrome.
