How should sternum fracture handle?
summary
The patients with sternal fracture had obvious chest pain, cough, aggravated pain when breathing and changing posture, shallow and fast breathing, weak cough and increased respiratory secretions. Deformities can be seen in the sternum. There are multiple costal cartilage fractures beside the sternum fracture, which may lead to flail chest due to sternum floating. Blunt heart injury, trachea, bronchus and intrathoracic vascular and branch injury are easy to occur. Deformity and obvious tenderness can be seen in the fracture site, with fracture displacement of the lower end forward and the upper end backward.
How should sternum fracture handle?
The treatment of simple sternal fracture is mainly bed rest, local fixation and analgesia to prevent complications. For local fixation, sandbags or small sternal splints were used.
The displaced sternal fracture should be reduced as soon as possible after the general condition is stable. Manipulative reduction under local anesthesia is often used to avoid violence combined with injury. After reduction, patients must rest in bed for 2-3 weeks; Patients with difficulty in manual reduction and floating sternum need open reduction and stainless steel wire fixation under general anesthesia. They should get out of bed early after operation.
Local fixation and immobilization: (1) for closed single rib fracture, multi head chest band, elastic chest band or half ring wide adhesive tape can be used for overlapping fixation( 2) For closed fractures of multiple ribs, sandbags or gauze pads can be used to stabilize the floating chest wall or instruct the patient to lie on the injured side.
